Evolve Rehab has contracts with ACC and Te Whata Ora. We require a referral from a funder to initiate the process. For a copy of the referral form that includes a list of other documentation/information required click the link below. Once completed email to referrals@evolverehab.co.nz

Referral FormFollowing the receipt of the required documentation a pre-admission assessment maybe carried out. This assessment can be completed at the facility or at an agreed location that best suits all parties. The pre-admission assessment is carried out by the multidisciplinary team with the client and their family/whānau and current support team.
