Respite Care
At Evolve Rehabilitation we offer respite care to allow caregivers a short break from their continuous caring responsibilities.
Respite care is available for ACC individuals and is on a case-by-case arrangement through our facilities. Our multi disciplinary team and funder work with the caregiver to understand the persons disability support requirements over the period they may stay in one of our facilities.
Respite care provides caregivers a short break from their continuous caring responsibilities. The client can stay for short periods up to three weeks each year allowing their caregiver a break.
Can anyone with a disability access respite care with Evolve?
Respite care at Evolve is not available for all people whorequire residential level of support. We provide respite care for individuals covered by ACC and is on a case by case basis. We work collaboratively with caregivers and funders seeking to place their loved one in respite care to determine if an Evolve Rehabilitation facility is the best option.
Process for accessing our respite care
We work with caregivers to determine if respite care at an Evolve Rehabilitation facility is right for the person in their care. Through an individual case assessment, we will determine the level of care required by our team, and any funding that may be available.
Caregivers will need to be able to provide individual care supplies for a client who is able to access our respite care services. You will need to bring:
• Medication
• Toiletries
• Customised adaptive equipment
• Medical consumables
• Clothing and other personal items
• Personal items such as phones or laptops should the client use these

ACC provides funded respite care packages. ACC case managers can directly contact the facility co-ordinator to discuss bed availability and any pre respite assessment that maybe required. Individual funding levels and agreements are reached before admission.
